WAIVER FORM

As a softball player I understand that injuries may occur while participating. In consideration for the right to play softball I sign this roster to waive liability and to indemnify Texas Softball Hall Of Fame, its owners, employees, leasing partners, officials, Board of Directors, ASA, its agents, and umpires any liability arising from my participation in any Texas Softball Hall Of Fame events. I waive and release all liability for injuries and/or death from any cause and from any bats or balls which are allowed in play for these Recreational events, including those not allowed for Championship play. I agree to only use alcohol responsibly and understand that any and all injuries, accidents, property damage, or deaths from my alcohol use are my sole and complete liability and not that of Texas Softball Hall Of Fame, its owners, employees, leasing partners, officials, Board of Directors, ASA, its agents, and umpires.

If any player fails to sign this form or to complete the information requested, he or she may be held accountable with consequences for eligibility and of liability.
